TERMS & CONDITIONS

These terms and conditions apply to all services, treatments, and memberships provided by Revitalis. By booking with us, you are agreeing to the following:

General Information

  • Revitalis is a private healthcare and wellness clinic offering physical therapy, osteopathy, performance-based rehabilitation, and related services.

  • All services are provided by qualified practitioners within their scope of practice.

  • We are Human Becomings Limited trading as Revitalis a company registered in England. Our company registration number is 15621729 and our trading address is 18 Saint John's Road SW11 1PN . 

Appointments

  • Appointments can be made via our online booking system, in person, by email, or by phone.

  • You will receive confirmation and reminders by email or SMS.

  • Please arrive at least 5 minutes before your scheduled appointment.

  • Please note that you must be at least 18 years of age to enter into a contract with us. 

Late Arrivals

  • If you arrive late, your appointment will still end at the scheduled time, and the full fee will apply.

  • We cannot guarantee full treatment time for late arrivals.

Cancellations & No Shows

  • Cancellations must be made at least 24 hours in advance.

  • Appointments cancelled with less than 24 hours’ notice will be charged at 100% of the service fee.

  • No-shows will be charged the full session amount.

Membership & Packages 

  • Memberships and packages are non-refundable and non-transferable unless otherwise stated.

  • Each has a specified expiry period and benefit structure, which is clearly outlined upon purchase.

  • Late cancellations will result in the loss of a session credit.

Payments

  • Payment is due at the time of booking or at the appointment.

  • We accept debit/credit cards, Apple Pay and other common forms of digital payment. 

  • Invoices can be issues for insurance purposes upon request.

Health & Safety

  • Please notify your practitioner of any health conditions, medications or injuries before your treatment.

  • Revitalis reserves the right to decline treatment if we believe it is not safe or suitable for your condition. 

Data & Privacy

  • All personal data is stored securely and handled in accordance with GDPR regulations. 

  • We will never share your information without explicit consent. 

Our right to refuse treatment

  • We reserve the right to refuse or discontinue treatment if:

 

  • A client is under the influence of drugs or alcohol.

  • There is abusive or inappropriate behaviour towards staff.

  • We deem the service to be clinically inappropriate.

Changes to terms

  • Revitalis may update these terms periodically. Any updates will be published on our website. 

By booking an appointment or purchasing a service/package, you agree to these terms.
